Vice President: Annette Toledo
I have been a nurse since 1977. I have worked as a leader in many of my roles. Working 12 years at Port Jefferson HealthCare Facility, now name Waters Edge. I continued my work life journey for 21 years at Brookhaven Health Care Facility where I was an Associate Director of Nursing and Education. I trained all new employees and managed the Workers Compensation Program saving the facility and company costly outcomes. I participated in Employee Health and Infection Control. Winning the Nursing Excellence Award during my tenure was so appreciated and displayed my loyalty and talents in many ways.
I then pursued my career into the Assisted Living world. As a Wellness Director of an ALF community for 1 year I was then promoted to Regional Director of Quality and Education of Wellness for 5 Wellness Departments for another 7 years.
My latest work life journey is now in Home Health Care where I find my greatest passion is assisting and transitioning patients form hospital or Subacute Rehabs to their homes. I get to see and talk with patients when they need help most until the time, they are ready to become independent again. I have earned an Elite status for the last 3 years in the Northeast region of the company and continually remain top in the country.
I am Vice President of the Long Island Nurses Honor Guard. We represent all nurses from Long Island that have passed and give them the ceremony they deserve for the many years of service.
I have created a woman’s group called W.R.E.N. Women Recognized and Empowering Network.
This is a safe place for professional women to get together and enjoy dinner and conversation. They can share work or personal experiences without the risk of being judged. I must say we have fun.
I would hope to be a positive influence for the CMSALI committee. This will be a great opportunity for my professional growth and be able to learn from many other professionals on the Case Management stage.
I have read the role and responsibilities of the Vice President and feel that I will be able to fulfil and surpass the expectations.

Secretary: Janice Thorn
After a long history as a sales executive in wholesale fashion Janice Thorn started working with geriatric patients 6 years ago. She worked alongside the physical, occupational and speech therapy teams at Fox Rehabilitation working with patients to regain their strength with therapies where they live. She is currently the Long Island Healthcare Account Executive with A Place for Mom helping families to explore and secure the best, most-appropriate senior living, memory care, respite or homecare solution for their loved ones next stage of life.
Janice thoroughly enjoys working with families to solve what is often an emotionally overwhelming change of life.
Janice is a very proud single-mother to a 19 year old daughter who attends college at Arizona State University. She enjoys writing, reading and anything near the ocean

Director: Corinne La Corte
I began my career in healthcare in 2013 as a medical assistant for Dr. Folan. After just a few months, I transitioned into the role of patient coordinator, assisting Dr. Folan in Suffolk rehabs and assisted living homes while managing the coordination of all medical services in the area. Later, I transitioned to Focus RX, a home infusion company, where I managed Nassau/Suffolk IV ABX and patient care. When the company closed, I returned to my passion for working with the geriatric population.
I began my next chapter as the Director of Memory Care at Somerset Gardens in Plainview. When my Executive Director, Dani Laino, transitioned to Sunrise Senior Living, I was offered the position of Reminiscence Coordinator at Sunrise Senior Living of West Babylon, where I continue to make an impact in memory care.
When I am not working, I volunteer once a month for the Alzheimer’s Disease Resource Center (ADRC) with Carol Hartman at Sisters of St. Dominic, providing them with a monthly craft activity. I am also an active member of the committee for WREN, a women’s empowerment networking group, where I am dedicated to uplifting and supporting women in their personal and professional growth.
Additionally, I feel I can play an intricate role as a member of the Case Management Society of America (CMSA), contributing my skills and experience to support best practices in patient care and advocacy.

Director: Donna Raye-Sullivan
Donna Raye-Sullivan has been a Professional Registered Nurse for 35+ years, having worked in various positions. Her educational background includes a BSN from Molloy College and a MBA in Health Care Management from Quinnipiac University. Donna has worked in Care/Case Management for the last 15 years and received her CCM in 2012. She also holds certifications in Managed Care Nursing (CMCN) and Nurse Manager and Leader (CNML). She has been a member in good standing of CMSA and CMSA LI since 2010. Currently working at Northwell Health as the Manager of Case Management for both Plainview and Syosset Hospitals.
Donna is a strong proponent of Care Management Certification and encourages her fellow care managers to expand their knowledge base and join their local/national professional organizations whenever possible. In addition to CMSA/CMSA LI, She is a member of the American Association of Managed Care Nurses and has contributed to multiple chapters in their educational textbook, A Nurse’s Guide to Managed Care Nursing, 4th Edition, American Organization for Nursing Leadership, American Care Management Association, and the American Nurses Association.
