Shelly Samuels has been in the Home Care industry since 2004 whether it be in home infusion or licensed home care in the private pay sector. In 2004 she worked for Advanced Care Inc. which was a home infusion company and then went to work for a licensed home care agency called Alvita Care as a Sales Director. Currently, she is working with Americare Pharmaceutical Services home infusion company. Shelly works with Case Managers and Social workers in her current role as Director of Infusion Sales for Americare Pharmaceutical Services. She works closely and collaboratively with Case Managers and Social workers in various sectors including hospitals, home care and managed care. Shelly is an active CMSA member for over 14 years and is currently the President for the Long Island chapter. In addition, she is an active member with the CMSA NYC chapter through attending their meetings and networking.

Past President: Donna Raye-Sullivan, BSN, RN, MBA-HCM, CCM, CMCN

Donna Raye-Sullivan has been a Professional Registered Nurse for 30 years, having worked in various positions. Her educational background includes a BSN from Molloy College and a MBA in Health Care Management from Quinnipiac University. Donna has worked in Care/Case Management for the last 15 years and received her CCM in 2012. She has been a Member in good standing of CMSA and CMSA LI since 2010. Currently working at Centers Plan for Healthy Living, Donna is the Clinical Manager for Complex Care Management.

Donna is a strong proponent of Care Management Certification and encourages her fellow care managers to expand their knowledge base and join their local/national professional organizations whenever possible. Most recent, she contributed to the American Association of Managed Care Nurses (AAMCN) publication A Nurse’s Introduction to Managed Care, 4th Edition, which prepares nurses for taking the Certified Managed Care Nurse (CMCN) examination.

She is a member of CMSA and CMSA LI since 2010. Donna has assisted with the CMSA LI Annual Conference for the last 2 years in the capacity of Welcoming and Sign-In. She has assisted at standard meetings at front table to check members in and to sign up new members. Donna has also attended the last 3 CMSA National Conferences and is registered to go to the one this year in Chicago.

In addition to CMSA/CMSA LI, She is a member of the American Association of Managed Care Nurses and has contributed to multiple chapters in their educational textbook, A Nurse’s Guide to Managed Care Nursing, 4th Edition.

Professional organizations that she belongs to, in addition to the above include National Association for Healthcare Quality (NAHQ); American College of Healthcare Executives (ACHE); American Nurses Association.

Elaine Halloran is a registered nurse for 40+ years with 28 years’ experience working in Case Management. She is graduate of Hunter College School of Nursing and received her MS in Health Care Management from Stony Brook University.

Elaine earned her CCM in 1996. Her professional experience includes Visiting Nurse for VNSNY, Ambulatory Case Management, Managed Care at Oxford Health Plan and for the past 20 years has worked in hospital Case Management. She has been the Director of Care Management at Good Samaritan Hospital Medical Center and currently is the Director of Care Management at St. Francis Hospital in Roslyn, NY.

For the last few years I have worked in the world of Big Insurance and have spent all of that time educating folks on specific and seemingly intricate insurance products and programs. It is that expertise that has brought me to my current role at Senior Planning Services, a full-service Medicaid application company, one that specializes in NY Chronic and Community Medicaid, both of which are the ones that many patients leaving a hospital setting, via the care and guidance of a case manager are in dire need of. My unique skill set enables me to educate case managers on how best to prepare and educate their patients on the Medicaid that is most applicable to them. I joined CMSALI this year for the purpose of bettering my relationships with case managers and I feel strongly that I have an obligation to be part of an organization that helps case managers. Just like case managers enable me to help the people of NY learn more about Medicaid so I would like to be part of the apparatus that helps case managers better themselves. It is quite simple for me, if my case managers are better at their jobs, they will ultimately do better at helping the very people that I have set out to help in my role at Senior Planning Services. I see my efforts to bring value to the CMSALI as a conduit to helping patients and am therefore overjoyed with an opportunity to serve as the Secretary of the CMSALI.

I work for Royal Certified Home Care, as an Account Executive managing Hospitals in the Nassau County Area, facilitating with home care coordination along with the Nurse Case Management department. I have over 15 years Marketing in this industry, in the tri-county area, and recently servicing Long Island. On a personal note, both living and working on Long Island, has been an absolute dream come true. I have recently participated in the annual symposium. Where I had already brought on 2 new members to the organization. I know my experience and expertise will be of value to CMSA LI as well as offer me personal growth and further my career. I look forward to being an active member of the board and help grow this wonderful organization

Director: Melissa Montgomery

Melissa is a nurse with over 20 years of experience. Her focus is in geriatrics. She is an active member of the CMSALI and also involved with GPNY, NAPIC, SUN and local chambers of commerce. Melissa has been in the healthcare field since her first job in high school working for a local Drs office filing charts. Over the years she has worked closely with the geriatric community in local nursing facilities as a medication nurse, unit charge nurse, and building supervisor. Now as a Clinical Community Liaison at Massapequa Center for Rehabilitation and Nursing she works closely with social workers, case managers, physicians, assisted living facilities, home care agencies, and elder care attorneys to help patients receive the care they need. Her goal is to continue to help patients and families while working collaboratively with other healthcare professionals.

Director: Karen Zinna, RN

Karen Zinna is a Registered Nurse and Certified Case Manager with over 25 years’ experience in Quality and Case Management. Her career spans a variety of health care settings including Care Management, Article 28 Ambulatory Care Clinics, Workman’s Compensation, Managed Care, Hospitals and Primary Care offices. She has an in-depth knowledge of health care operations at all levels with proficiency in the development and training of case management programs. She was the former Director of Care Coordination and CM at Advantage Care Physicians and is currently a Healthcare Consultant with a private firm that specializes in assisting physician practices to obtain the status of the Patient Centered Medical Home. Karen has been a member of CMSA for over 10 years.

Director: Jennifer Devine, LMSW

Jennifer Devine is a licensed Social Worker and Home Care Consultant for Caring People Home Care, a licensed HC agency. She has been a practicing Social Worker since 2000. She started her career in the psychiatric field and moved into the geriatric field in 2004 and feels this is her passion. She is a member of CMSA LI in good standing and will bring her social work perspective into the organization.

Director: Andrea Cowles, RN, BSN, MA, CCM

Andrea is a registered nurse with over 25 years of nursing and management experience. She has been an active member of the CMSA Long Island Board of Directors and has served as Vice President.

Andrea began her career as a public health nurse before joining Visiting Nurse Service and Hospice of Suffolk in 1990, where she is currently the Director of Intake and Service Development. Prior to joining VNSHS, she was a Clinical Care Manager for Family Care Certified Services of Nassau and a Community Health Nurse at North Shore Long Island Jewish Health System.

Andrea holds a Bachelor of Science in Nursing from the State University of New York at Plattsburgh, a Masters of Art in Health Administration from Hofstra University and is a Certified Case Manager. She is a member of the Case Management Society of America*, the Senior Umbrella Network of Nassau County and Huntington Town Women’s Advisory Council.

Annette has been a nurse since 1977 and has worked as a leader in many roles. Currently in Home Health Care where she finds her greatest passion in assisting and transitioning patients from the hospital or subacute rehabilitation facilities to their homes that are ready to become independent again. In addition, Annette is the Vice President of the Long Island Nurses Honor Guard which represents all nurses from Long Island that have passed and gives them the ceremony they deserve for their many years of service. Annette has created the woman’s group W.R.E.N., Women Recognized and Empowering Network. This is a safe place for professional women to get together and enjoy dinner and conversation without the risk of being judged.
